Our Services
Level 1,2 & 3: Support Connections, Coordination & Specialist Support Coordination
Three levels of support coordination can be included in NDIS plans:
- Support Connection – This support focuses on building the participant's ability to connect with informal, community, and funded supports, maximising their plan, and promoting the pursuit of their goals.
- Support Coordination – This support assists participants with building the skills needed to understand and manage their plan. A support coordinator will work with participants to ensure a variety of supports are utilised to increase the participant's capacity to maintain relationships, manage service delivery tasks, live more independently, and be included in their community.
- Specialist Support Coordination (SSC) – This is a higher level of support coordination, accessible for participants with complex situations that require specialist support. An SSC will support participants to manage challenges in their support environment and ensure the consistent delivery of service.
